Phone Triage Nurse (RN)

We're currently hiring for a contract-to-hire opportunity for an RN within a community health center in the Swissville area of Pittsburgh, PA.

Location: Pittsburgh, PA

• Position starts out onsite fully.
• After six months or upon transition to permanent, it offers a hybrid schedule with one day per week remote.

Schedule: Monday-Friday, 8:00 AM to 4:30 PM Monday to Friday, with one evening shift on Wednesdays each week from 10:30 AM to 7:00 PM.

Pay Rate: $30.29 to $34.00/hour, depending on experience.

Key responsibilities include:

• Triage of patient calls and inquiries.
• Patient education on various topics, such as medication, chronic conditions, and transgender care.
• Coordination of patient appointments and follow-up care.
• Collaboration with the internal healthcare team to ensure comprehensive patient care.

Requirements:

• Active Pennsylvania Registered Nurse (RN) license.
• At least one (1) year of nursing experience within either Med-Surg, Critical Care, or Home Health. Previous triage or case management experience is preferred.
