Job description

Job Summary:
Support patients through the healthcare system as an advocate and navigator. Coordinate care, facilitate education, and promote self-management for patients, especially high-risk individuals. Ensure seamless transitions between providers, hospitals, and specialty care.

Key Responsibilities:

  • Guide patients through the healthcare system, acting as an advocate.
  • Participate in team meetings, quality initiatives, and clinical audits.
  • Provide health education and support self-management for chronic conditions.
  • Coordinate care between providers, hospitals, and external facilities.
  • Assist with triage, medication management, and preventive screenings.
  • Ensure clear communication among care teams and treating clinicians.
  • Collect and analyze biometric data for population health initiatives.
  • Support MAs with clinical guidelines and protocols.

Requirements:

  • Active California RN license, cannot reside in the state of California
  • BSN required; multi-state licensure is needed, must reside in a compact state
  • 2-5 years of outpatient clinical experience (chronic disease management preferred).
  • Strong verbal communication and computer proficiency (Google Suite, EHR).
  • Ability to work occasional evenings and weekends.

Schedules: (PST)

  • 7:00 AM - 6:00 PM | Rotating weekdays & weekends.
  • 8:00 AM - 7:00 PM | Varied schedules including weekends.
  • 9:00 AM - 8:00 PM | Rotating weekdays & weekends.

Pay:

  • $36 - $40 per hour
