Job Title: Nurse Practitioner (NP) - Home Health

Location: South Bend, Indiana

  • Traveling throughout North West Indiana - Merriville, Gary, Crown Point, Schereville, Griffith, and Munster

Salary Details:

  • $115K-$135K Annual Salary - depending on experience

Shifts - Full Time Hours

  • M-F 8am-5pm
  • No weekends
  • On a call rotation every 5th week

Contract Type - Contract to Hire

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our dynamic team working at a Home Based Primary Care Practice. The NP will provide high-quality care to Geriatirc patients performing assessments, diagnoses, and treatments under the guidance of established clinical protocols. This role is ideal for an NP who thrives in an autonomous environment, values holistic patient care, and is passionate about improving the quality of life for older adults.

General Overview:

We are seeking a skilled and compassionate Nurse Practitioner to join our team in providing home health primary care services. The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

Essential Functions:

Patient Care:

  • Provide comprehensive primary care services to geriatric patients, including assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions.
  • Conduct thorough health evaluations, including physical exams and medication reviews.
  • Develop individualized care plans in collaboration with patients, families, and caregivers.

Home Visits:

  • Travel to patients' homes to deliver medical care, ensuring a comfortable and safe environment.
  • Identify and address environmental and social factors impacting patient health.

Collaboration:

  • Work closely with interdisciplinary teams, including Medical Assistants to coordinate patient care.
  • Communicate effectively with family members and caregivers to provide education and support.

Documentation and Compliance:

  • Maintain accurate and timely medical records.
  • Ensure compliance with all applicable healthcare regulations and organizational policies.

Job Requirements

  • Master's or Doctorate degree in Nursing from an accredited program.
  • Current, unrestricted Nurse Practitioner license and certification in the state of Indiana
  • Federal DEA License/eligibility
  • Active CPR/BLS Certification
  • Minimum of 2 years of clinical experience as a Nurse Practitioner in Primary Care, Home Health, (preferred) or Hospice required
  • Experience/Comfortable with providing care for senior/geriatiric populations
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Comfortable traveling into patients homes
