Job Title: Registered Nurse (RN)

Overview: The Registered Nurse Care Manager (RNCM) is a critical part of the health care team and is responsible and accountable to the patients. The RNCM assists clinicians in the development of the plan-of-care for patients. In this role the RNCM is responsible for the care management and coordination of a patient panel and act as an advocate, educator and resource person for this population.

Oversees the care plan for the patient based on clinician recommendations
Communicates the personalized plan of care to the patient/family/significant other
Ensures, to the degree possible, the patient's adherence to the care plan via guidance/education
Provides/oversees coordination of services for the patient based on the diagnosis and the plan of care developed by the clinical team.
Performs in-depth screening of enrollees for disease and/or case management eligibility, using predetermined criteria as outlined in the disease case management program.
Completes a nursing assessment/note in the patient's chart after face-to-face interactions, telephone conversations and/or clinical team meetings.
Provides preventative and disease-based education based on the diagnosis and or any needs identified during pre-visit huddles, Rounds, etc.
Follows up on referrals, imaging, laboratory testing, specialty referrals, and other external encounters as recommended by the clinical team
Presents pertinent clinical, behavioral, or social patient information to the team during clinical team meetings or as requested.
Maintains ongoing communication with Physician and health plan providers regarding enrollee's condition and response to care.
Collaborates with the Community Health Worker (CHW) to enhance identification of barriers to care and reports to the clinicians and their teams.
Works with the patient to encourage self-management of medical conditions/care
Monitors the Health Information Exchange (i.e. CRISP) to identify high risk patients or patients
- Active RN License.
- Ideally, 1-2 years of experience in an outpatient setting preferred. Candidates with experience in inpatient acute care or case management will also be considered.
Schedule:
- Full-time, 40 hours per week.
- Monday to Friday, 8:00 AM - 4:30 PM. one late day 10-7pm
Compensation:
- Pay Range: $40-45/hr
