Job Description

Responsibilities/ Job Duties:
As a Case Manager for DC Medicaid, you will support enrollees, many of whom have behavioral health issues and experience poor social determinants of health. You will be responsible for conducting needs assessments, assigning acuity levels to enrollees, and determining the frequency of contact based on their individual needs.

  • Acuity Levels:
    • Level 1: Well-managed enrollees with minimal Medicaid use.
    • Level 2: Moderate Medicaid use, needing more frequent attention.
    • Level 3: High utilizers with complex needs, requiring more frequent touchpoints.

Your role will include providing case management services, conducting home visits, developing care plans, and collaborating with primary care providers to ensure the best outcomes for enrollees.

Typical Case Management:

  • Start with a needs assessment and assign an acuity level to determine how often the care manager needs to follow up with the enrollee.
  • Conduct regular field visits (3 per week) and ensure thorough documentation.
  • Attend well-child visits with enrollees when possible to update and support care plans.
  • Break down socio-economic barriers and support enrollees and their families in navigating the healthcare system.

What does the caseload look like?

  • Standard Case Managers: 75 patients across levels 1, 2, and 3.
  • Specialty Case Managers: 60 patients with complex needs (level 3 acuity).
  • Field visits count towards office days for documentation, but flexibility is provided in scheduling.
  • Expect 3 care plans or assessments completed each week, with 3 field visits scheduled.

Minimum Education and Experience Qualification Requirements:

Required:

  • Active D.C. RN License
  • At least 2 years of experience in case management or a related field
  • Experience working with Medicaid, pediatric, or behavioral health populations
  • 5+ years of experience in a similar role preferred
  • Experience with complex case management and Medicaid populations preferred
