Job Description

This role is based in a very small facility with:

• 25 nursing beds
• 25 boarding home independent living residents
• 25 independent living residents

The LICSW will oversee Medicaid reviews, admissions, discharges, and transfers, as well as MDS sections and care planning. They will serve as the main point of contact for families and residents and be responsible for the overall management of the facility. Additionally, the role includes acting as an abuse and grievance counselor in collaboration with administration for required reporting.

Responsibilities/ Job Duties:

• Review care plans and new information upon arrival each day
• Return all phone calls related to care and facility needs
• Prepare quarterly reports and update care plans
• Coordinate and conduct interviews for new admissions
• Collaborate with hospitals for discharges (primarily for hospital transfers)
• Work closely with residents, as most are long-term/lifetime residents

Minimum Education and Experience Qualification Requirements:

• Licensed Independent Clinical Social Worker - LICSW (Required)
• Experience working with long-term care/elderly patient populations required
• Experience establishing care plans required
• Experience with MDS process for clinical assessment (Medicare/Medicaid patient populations in care facilities) required
• Must be comfortable working in an environment with Christian affiliations
