Job description

Job Posting: RN Medicaid Care Manager

Location: Washington, DC
Schedule: Monday - Friday, 8 am - 5 pm (hybrid/ flexible scheulding 3 in person visits a week)

Position Overview

We are looking for dedicated, empathetic Care Managers to join our Medicaid team, supporting DC Medicaid enrollees, many of whom face behavioral health challenges and difficult social determinants of health. This role is integral to our mission of providing compassionate care management and breaking down barriers to improve overall patient well-being.

Key Responsibilities

  • Patient Assessments: Conduct needs assessments with each enrollee to establish a clear understanding of their health and social circumstances
  • Caseload Management: Start with a manageable caseload of 30 enrollees, working up to a full caseload of 75 patients with varying acuity levels.
  • Care Planning & Coordination: Create and update individualized care plans. Conduct three care plan assessments weekly through field visits to enhance patient outcomes and support patient well-being. Documentation and follow-up work are completed in-office.
  • Relationship Building: Work closely with enrollees, families, and primary care providers, attending well-child visits as needed. Develop strong relationships with providers to enhance care coordination and support member needs effectively.

Qualifications



Must-Have

  • Licensure: RN in DC
  • Experience: 2 years RN exp, Case Management exp preferred
