Remote Pharmacy Technician

Job description

Schedule: 6am-230pm OR 9am-6pm PST ( you will need to match PST hours regardless of the time zone you sit in )

What You'll be Doing:

  • Medication Reconciliation:

Perform the medication reconciliation process, including verifying the patient's current medications, clarifying dosage and appropriateness, reconciling discrepancies, and transmitting the updated list to the appropriate healthcare providers.

  • Clinical Issue Identification:

Identify potential medication-related issues (e.g., duplications, omissions, interactions, dosing errors) and escalate concerns to a clinical pharmacist for further review and resolution.

  • Patient Education:

Explain the benefits of clinical programs and services, providing guidance on medication management, and supporting members in understanding their treatment plans.

  • Collaborate with Providers:

Obtain provider contact information to facilitate seamless delivery of clinical interventions and ensure the accuracy of medication regimens during transitions of care.

  • Accurate Record Keeping:

Maintain thorough documentation in the software platform, ensuring all interactions and clinical details are accurately recorded.

  • Follow-up & Coordination:

Follow up with hospitals, clinics, pharmacies, and patients to resolve any issues or concerns, ensuring timely and effective care coordination post-discharge.

  • Schedule Clinical Consultations:

Coordinate scheduling of telephonic or video appointments between patients and clinical pharmacists for additional support and intervention.

  • Process Improvement:

Participate in team meetings to identify opportunities for process improvement and streamline workflows for more efficient service delivery.

Additional Responsibilities:

Assist with obtaining referrals and authorizations as needed, and participate in other assigned projects to support the team's goals.

Required Qualifications:

  • State Licensed Pharmacy Technician
  • National Pharmacy Technician Certification (PTCB or ExCPT)
  • Medication Knowledge: Familiarity with basic medical terminology and both brand and generic medication names.
  • Communication Skills: Excellent written and oral communication skills to effectively engage with patients, providers, and team members.
  • Computer Proficiency: Strong computer skills, including proficiency with Microsoft 365, Google Workspace, and MacBooks.
  • Self-Motivation: Ability to prioritize tasks and work independently in a fast-paced, remote environment.
  • Work Schedule: Ability to commit to a 40-hour work week.

Preferred Qualifications:

  • Experience working with electronic medical records (EMRs) or healthcare platforms.
  • Prior experience in an ambulatory care setting, particularly in taking medication histories and facilitating medication reconciliation.
  • Experience in a high-volume healthcare environment.
  • Bilingual proficiency is a plus.

Remote Work Requirements:

  • A private, secure workspace that ensures HIPAA-compliant information privacy.
  • A stable, high-speed internet connection for telephonic and video work.
