Job description

Medix is looking for a self-motivated and passionate REMOTE MLTC RN for our Full-Time Care Manager opening. This is an exciting and dynamic position, as the Care Manager, you will be responsible for ongoing assessment of health, environmental, and psychosocial status of members assigned to you, and ongoing care management of members through telephonic contact.

What you'll do:

  • Some of your daily activities will include all forms of care management, such as- care planning, coordination, and assessment of health.
  • You will monitor services for quality, effectiveness, and progress toward goals.
  • Coordination of transitions for patients including discharge planning from the hospital or nursing home.
  • Identifies health, environmental, and psychosocial risks for assigned members and recommends appropriate interventions to the Interdisciplinary Care team

Qualifications:

  • NY RN active license
  • MLTC expereince
  • Case maangment experience
  • Bilingual in Chniese/Spanish is a plus!

Pay: 80,000-100,000 (based on experience)
