Job description

The Registered Nurse (RN) assigned to these areas provides direct and indirect nursing care by contributing to the patient's assessment, determining priorities, identifying nursing measures and therapeutic objectives, and evaluating outcomes. The position requires independent judgment and nursing actions, flexibility, organizational skills, and the ability to utilize an interdisciplinary approach in the provision of nursing care.
The specific duties and responsibilities of these RNs include:

Office of Community Care (OCC) Registered Nurses

  • Clinically review consults/requests for completeness and appropriateness/utilization review.
  • Disposition care in the community per utilization review and guidelines.
  • Clinical review of incoming medical records, documenting communication with ordering providers and primary care providers (PCP).
  • Coordination of care, including corresponding with veterans, community providers, internal providers/nurses, and administrative staff to ensure that the requested care and authorization for care are processed.

Referral Coordination Team (RTC) Registered Nurses

  • Triage specialty consults for clinical appropriateness and completeness within the National RCI Standards for coordinated care.
  • Communicate with veterans regarding their health care options via phone calls to determine whether they will receive care at a VA facility or through community-eligible providers, allowing veterans to make informed decisions on how, when, and where to receive specialty care if eligible for Community Care (CC).
  • Provide clinical knowledge to veterans and act as a clinical resource to multidisciplinary staff (MDs, LIPs, SW, HAS, etc.) within VISN19 and outside the VISN.
  • Collaborate with CACs to rebuild/modify consult templates.
  • Serve as a liaison to both internal staff and Community Care (CC) staff during the consult process.
  • Act as a liaison to the ordering provider and specialty care services where RCI is implemented.
  • Serve as Delegation of Authority by building trusting relationships with specialty providers in the clinics where RCI is implemented, based on vetted Referral Coordination Team (RCT) Specialty Triage Tools in collaboration with VA specialty providers.

Must Have Skills/Qualifications:

  • Graduation from a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The commission on Collegiate Nursing Education (CCNE).
  • A current license in at least one (1) state, territory, or commonwealth in the U.S. or the - District of Columbia.
  • A minimum of two (2) years direct patient care experience within the last three (3) years.
  • Current Basic Life Support certification.
  • A current CV which identifies the provider's education and professional qualifications commensurate with the position or specialty for which they are being offered to perform.

Schedule/Shift: Hours of operation are 0730 - 1730, Monday-Friday, excluding holidays.

Services shall be provided on-site, at the following locations:
1. Glendale, CO 80246
2. Aurora, CO 80045
