Job description

Job Summary:
The incumbent supports Veterans, their families, caregivers, and other stakeholders, ensuring courteous, respectful communication and service. They provide consistent information based on VA policies and handle conflict constructively. The position requires sensitivity to the unique needs of Veterans, including those aged 40-50 and geriatrics (60+), with occasional younger patients.

The incumbent is responsible for safeguarding sensitive data, ensuring compliance with the Privacy Act, and reporting any security or privacy incidents immediately. Non-compliance or data breaches are unacceptable.

General Responsibilities:
The Registered Nurse (RN) provides direct and indirect nursing care, including patient assessments, prioritizing care, identifying nursing measures, therapeutic objectives, and evaluating outcomes. The role demands independent judgment, flexibility, organizational skills, and the ability to collaborate in an interdisciplinary approach to care.

Organizational Relationship:
The RN works under the professional direction of the Nurse Manager or designee during the workweek.

Duties and Responsibilities:

Office of Community Care (OCC) Registered Nurses

  • Review consults/requests for completeness and appropriateness.

  • Disposition care in the community per utilization review and guidelines.

  • Clinical review of incoming medical records, with documentation of communication with ordering providers and primary care providers (PCPs).

  • Coordinate care by working with veterans, community providers, internal providers/nurses, and administrative staff to ensure care and authorization processes.

Referral Coordination Team (RTC) Registered Nurses

  • Triage specialty consults for clinical appropriateness and completeness within National RCI Standards.

  • Communicate with veterans about their healthcare options, enabling informed decisions on receiving care through VA or community providers.

  • Serve as a clinical resource to multidisciplinary staff within VISN19 and beyond.

  • Collaborate with CACs to rebuild/modify consult templates.

  • Liaise with internal staff and Community Care (CC) during the consult process.

  • Act as a liaison between ordering providers and specialty care services where RCI is implemented.

  • Build trusted relationships with specialty providers based on Referral Coordination Team (RCT) Specialty Triage Tools in collaboration with VA specialty providers.

Qualifications:

  • Registered Nurse (RN) with a degree from an accredited nursing program (ACEN or CCNE)

  • Active RN license in a U.S. state, territory, or District of Columbia

  • Minimum of 2 years of direct patient care experience within the last 3 years

  • Current Basic Life Support (BLS) certification

  • Updated CV with relevant education and qualifications
