Job Overview:

• The Social Worker will be working with the Pediatric population
• Schedule: Monday - Friday 8:00 AM - 5:00 PM
• Pay: 70,000 - 85,000

Qualifications:

• LMSW or LCSW
• Open to New Graduates with internship experience in a hospital setting
• Experience in a healthcare setting is highly preferred
• Familiarity with local resources in Baltimore
• Pediatric experience is preferred, but not required

Overview of Responsibilities:

• Oversee pediatric Med/Surg patients, ensuring they have the necessary resources and support for effective discharge planning.
• Manage a caseload of up to 100 beds on the Med/Surg unit, with typical focus on 25-30 pediatric patients at a time.
• Provide in-hospital support, conducting visits in patient rooms or virtually for recently discharged patients who need assistance identifying and utilizing community resources.
• Conduct psychosocial assessments for pediatric patients and their families, addressing social, emotional, and environmental needs.
• Actively participate in interdisciplinary rounds with providers, utilization review teams, care managers, and social workers.
• Focus heavily on discharge planning, ensuring that pediatric patients and their families are well-prepared for discharge with the necessary follow-up and resources.
• Collaborate with community resources and make referrals as needed.
