General Overview:
We are seeking dedicated and compassionate Nurse Practitioners (NP) to join our Healthcare Team. As a Nurse Practitioner (NP) you will be responsible for conducting in-home visits, health assessments, and care planning for members with acute and chronic conditions. The position requires traveling through the Great Burlington area including Barre, Montpellier, Dover, and Brattleboro. The position offers a flexible schedule between Monday-Friday 8am-5pm. The ideal candidate is a team player with strong decision-making skills, who thrives in a collaborative, patient-centered environment.
Essential Functions:
- Conduct thorough review of available member records in preparation for scheduled visit to assist in supporting capture of appropriate diagnoses
- Utilize critical thinking to interpret diagnostic studies and screening evaluations and develop appropriate diagnosis and plan based on results
- Order, perform, and interpret diagnostic tests and communicate results timely with member and provider
- Develop, implement, evaluate, and alter treatment plan of care as needed
- Make recommendations and/or adjustments to medications as needed
- Communicate and collaborate effectively with physicians, other health professionals and health related agencies to promote continuity of care for member
- Manage general medical and surgical conditions based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of relevant treatment modalities and pharmacologic agents
- Demonstrate and participants at an advanced level of nursing knowledge with interdisciplinary care team to assure safe, effective, quality member care
- Participate in development of the Home Wellness Program to meet the health care needs of members
- Participate in development of Palliative Care Program to meet the health care needs of identified population
- Provide comprehensive care and symptom management for members enrolled in Palliative Care Program
- Demonstrate and maintain accurate, complete, and timely documentation in electronic health record to include but not limited to:
- Correct diagnosis code(s)
- Status of all acute and chronic conditions identified
- Plan to address all diagnosis identified at time of visit
- Demonstrate understanding and working knowledge of key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) model within 90 days of hire
- Demonstrate understanding and working knowledge of key elements of Quality STAR/HEDIS measures within 90 days of hire
- Participate in quality improvement activities and contribute to ongoing development of care improvement
- Function as clinical resource in collaboration with member care team and serve as an internal resource for clinical and non-clinical staff to promote positive interactions in support of member care and outcomes
Job Requirements
- Active Nurse Practitioner License in Vermont
- Current and valid State RN license required.
- Advanced Practice Provider with a minimum of 1 year experience working with adult, geriatric populations
- Reliable transportation and a verifiable good driving record. Must be in Burlington Area, able to work from home, and able to travel to patient homes, SNL, Healthcare Facilities.
- Must be self-motivated and able to perform work in accordance with defined operating procedures.
- Ability to develop and maintain inclusive and equitable relationships with patients, caregivers and staff
- Experience working with community resources from diverse backgrounds
- Ability to communicate effectively both orally and in writing.
- Must be comfortable interacting with patients and care teams both telephonically and in-person.
- Priority will be given to candidates who live in the communities we serve and whose life experience reflects that of our patient population
- Available to work full time hours Monday through Friday between 8am-5pm
