Job description

Position Overview:

We are seeking dedicated Social Workers to join our team and provide compassionate care in accordance with the policies of the VA Healthcare System. Social Workers play a crucial role in ensuring the well-being of our patients, offering support, assessment, intervention, and resources to address their diverse needs.

Key Responsibilities:

  1. Biopsychosocial Assessments: Conduct thorough assessments to understand the holistic needs of each patient.
  2. Social Determinants of Health: Identify and address social factors impacting patients' health and well-being.
  3. Suicide Risk, Mental Health, and Crisis Intervention: Conduct assessments and provide timely intervention and support.
  4. Documentation: Record patient care, safety plans, and other pertinent information accurately and comprehensively in medical records.
  5. Advanced Directives: Assist patients in completing advanced directives and understanding their options.
  6. Inpatient Psychiatric Care: Guide patients through the process and documentation required for inpatient psychiatric care.
  7. Interdisciplinary Communication: Collaborate with healthcare providers and teams to ensure coordinated care.
  8. Reassessment and Plan Revision: Continuously evaluate patient needs and adjust care plans accordingly.
  9. Resource Provision: Offer resources and assistance with transportation, housing, and other needs.
  10. Family and Care Meetings: Facilitate meetings to discuss goals of care and involve families in decision-making.
  11. Homelessness Support: Provide assistance and resources for homeless veterans.
  12. Abuse and Neglect: Identify and address instances of abuse or neglect, consulting relevant agencies as needed.
  13. Discharge Planning: Assist in planning and arranging appropriate post-discharge care and support services.
  14. Emergency Response: Initiate and prioritize appropriate measures in emergency situations.
  15. Case Management: Identify patients in need of ongoing case management post-discharge.
  16. Patient Education: Assess patient/family knowledge and provide instructions as necessary.
  17. Emergency Measures: Administer life-saving measures in the absence of physicians.
  18. Documentation Validation: Ensure the accuracy and completeness of all prepared reports.
  19. Professionalism: Maintain a professional appearance and demeanor at all times.
  20. Customer Service: Demonstrate helpful, courteous, and respectful conduct in line with VA standards.
  21. Privacy: Respect and uphold patient confidentiality and privacy rights.
  22. Record Review: Thoroughly review relevant information in electronic medical records.

Licensing Requirements:

All staff/facility personnel must be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Contractors' employees must hold full, current, and unrestricted licenses/certifications, without any past or present restrictions, suspensions, revocations, or limitations. Current Curriculum Vitae must be provided for review to ensure appropriate placement based on qualifications and experience.
