Job description

KEY DETAILS:

  • Experienced Intake Care Coordinator needed. Candidate must know HCPC, ICD10 & CPT codes.
  • Will be responsible for taking Infusion Therapy and Home Health referrals from referral sources.
  • Must be capable of verifying benefits, entering patient demographics into the computer system, communicating with other departments regarding the status of referrals, and notifying patients/families regarding coverage and payment responsibilities.
  • Ensures that insurance verification is completed, and authorizations are obtained before giving the referral to the Pharmacist.
  • Work Schedule: In office. Saturday to Wednesday, 9 am to 6 pm
  • Position can be remote, in-office, or hybrid.
  • Equipment will be provided by the client.
  • Contract-to-Hire opportunity.

QUALIFICATIONS:

  • Minimum Education: High School Diploma and/or Associate's Degree
  • 1-3 years minimum experience
  • Computer literacy, database experience, knowledge of general office equipment
  • ICD-9 experience helpful. Knowledge of medical terminology
  • Must possess above-average knowledge of numeric and alphabetical filing systems.

RESPONSIBILITIES:

  • Completes job duties within required time frame, according to established priorities or workflow requirements
  • Receives initial call on all new referrals, does data entry, verifies insurance, and obtains initial authorization and coordinates intake process with Pharmacy and Nursing department. Reports status of new referral to billing department.
  • Maintains liaison with the Pharmacy Department, communicates new referrals to Pharmacy/Nursing in a timely manner.
  • Enters initial orders in CPR system.
  • Maintains positive relationships with all referral sources.
  • Responds promptly to internal and external customer inquiries and requests.
  • Maintains intake & consents logs.
  • Works to develop a positive rapport with all staff members.
  • Communicates pertinent information regarding customer/client changes to appropriate team members.
  • Follows company policies and procedures.
  • Performs other duties as assigned by Director of Reimbursement.
