Hybrid RN/LICSW/LGSW Case Manager

Hybrid Care Manager Opening in Washington, DC

Are you still currently in the job market? We are hiring for Case Managers to work for a large healthcare organization in Washington DC. This position is open to RNs or LGSW OR LICSW with DC License!

Requirements:

• 1+ years of case management experience
• Must be vaccinated against Covid-19
• Must be Licensed in DC
• MUST BE OK WITH FIELD WORK AS NEEDED

Job Information:

• Will be working with patients with special health needs, from newborn to young adults (26)
• Will be performing needs assessments to determine an acuity level for the patient
• This position is a hybrid role where there will be 1 in person meeting with the team a month. Field work is as needed based on acuity of patients
• Will be working with enrollees and families/caretakers to break down any barriers that could come in between them and their treatment plan
• The majority of the work done in this role will be remote, however the Care Manager will be performing occasional field and in office visits

Schedule:

• Typically normal business hours
• Lot's of flexibility (you will be managing your own schedule)

Pay:

RN: 42-47/hr

LGSW: $32/-36/hr

LICSW: $38-42/hr

Based on experience + years with license!
