Job description

Join our team of Care Coordinators dedicated to providing comprehensive support to adults and children struggling with mental illness. As a Care Coordinator, you'll play a vital role in advocating for and supporting clients with complex medical and substance abuse disorders, ensuring they receive the services necessary for wellness self-management and reduced hospitalizations.

Your responsibilities will include:

  • providing clinical support
  • conducting thorough assessments
  • maintaining accurate records
  • facilitating access to community resources.
  • You'll engage in face-to-face outreach and crisis intervention, serving as a crucial link to essential services outlined in our interdisciplinary care plans.

To excel in this role, a Bachelor's Degree in relevant fields such as Social Work or Psychology is preferred, along with one year of human services experience. Excellent interpersonal skills and a willingness to travel are essential, while knowledge of entitlement programs and fluency in languages such as Spanish, Mandarin, Russian, Creole, Hindi or Benagli are advantageous.
