Job description

Join our team as a Case Manager and make a real difference in the lives of those in need! As a Case Manager, you'll play a pivotal role in providing and coordinating essential client services, ensuring their journey towards stability and empowerment. Here's what you'll be doing:

Essential Duties & Responsibilities:

  • Rapid Response: You'll conduct intake processes promptly, ensuring new clients receive immediate support and understanding of their rights and grievance procedures.

  • Assessment & Documentation: Perform thorough assessments and document client progress towards securing permanent housing, ensuring accurate and timely records of supportive services.

  • Housing Assistance: Prepare housing applications and guide clients through the application and interview process, easing their transition to stable accommodation.

  • Supportive Services: Monitor and assess client behavior and appearance, providing individual and group counseling, workshops, and recreational activities to promote holistic well-being.

  • Client Well-being: Regularly assess personal hygiene, room cleanliness, and daily living skills, ensuring clients receive the care and support they need.

  • Comprehensive Case Management: Develop and review service plans tailored to each client's needs, providing referrals for medical, mental health, and social services as necessary.

  • Emergency Response: Be prepared to intervene during crises and emergency situations, ensuring the safety and well-being of clients at all times.

  • Compliance & Documentation: Maintain accurate and up-to-date client records in compliance with regulatory standards, ensuring seamless discharge and transfer processes.

Requirements:

  • Education & Experience: A Bachelor's degree in Human Services is preferred, though a high school diploma or GED is required. You should have at least three years of experience working with the homeless population.

  • Diversity & Cultural Competency: Ability to work effectively with a culturally diverse population, demonstrating sensitivity and understanding.

Preferred Qualifications & Skills:

  • Local Knowledge: Familiarity with the NYC public school system is preferred, along with experience in residential settings, group work, case management, and community outreach.
