Bilingual Nurse Care Manager (Hybrid) (RN)

Medix is looking for a self-motivated and passionate RN for our Full-Time Care Manager opening. This is a Hybrid role! The candidate must be bilingual speaking Spanish.

This is an exciting and dynamic position, as the Care Manager, you will be responsible for ongoing assessment of health, environmental, and psychosocial status of members assigned to you, and ongoing care management of members through telephonic contact and occasional field visits.

Duties:

• Some of your daily activities will include all forms of care management, such as- care planning, coordination, and assessment of health.
• You will monitor services for quality, effectiveness, and progress toward goals.
• Coordination of transitions for patients including discharge planning from the hospital or nursing home.
• Identifies health, environmental, and psychosocial risks for assigned members and recommends appropriate interventions to the Interdisciplinary Care team.

Specifics:

Hours: Mon-Fri 9am-5pm (FT only)

Hybrid: 4 days Remote, 1 day in the field (Queens)

Field Location(s) available: Queens

Pay: $95,000 - $115,000

Job Type: Direct Hire

Requirements:

• NYS RN License along with 1 years job related experience in Managed Care, Home Care or Community setting.
• Bilingual speaking (Spanish)
• Seeking those who bring an Associate's degree or higher.
• Passionate attitude, communication skills and problem solving will be vital to this position.
