Job description

Overview: Seeking a licensed (LPC or LCSW) Behavioral Health Consultant to join a growing medical clinic in Houston, Texas. Qualified candidates will be bilingual in Spanish and have clinical experience working with women and/or children. Summary:

The Behavioral Health Consultant, as part of the primary care treatment team, identifies, triages, and manages patients with medical and behavioral health problems within the primary care setting. In addition, the Behavioral Health Consultant will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Will be working with women and children, children from the ages of 1 to 18. Prior experience working with children is required.

Job Duties:

  1. Assist primary health care providers in recognizing and treating mental disorders and psycho-social problems.
  2. Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
  3. Work with primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
  4. Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration.
  5. Identify, refer and serve as a liaison to the counseling team for patients that could benefit from longer term counseling services as appropriate.
  6. Assist in preventing relapse or morbidity in conditions that tend to recur over time. 7. Evaluate patient care plans with primary care team.
  7. Work with the primary care provider to refer cases to outside psychiatry as appropriate and maintain necessary follow-up / follow along care.
  8. Teach patients, families, prevention, and treatment enhancement techniques. 10. Document patient encounters in the Electronic Medical Record in a timely and thorough manner.
  9. Attend and participate in Behavioral Health / Counseling team meetings, as well as Quality Improvement activities as required.
  10. Responsible for self-care to ensure optimal, ethical and safe health delivery service to patients.

Qualifications, Education and Experience:

  1. Degree and current license as a Professional Counselor (LPC) or is Social Work (LCSW)
  2. Demonstrated ability to work comfortably with Hispanic patients in their native language and with an understanding of their cultural heritage

Preferred Qualifications:

  1. Prior work experience as a LPC or LCSW in medical setting or Federally Qualified Health Center
  2. Prior work with diverse patient population
  3. Familiarity or work experience in integrated behavioral health models.
  4. Prior experience with Electronic Medical Records (EMR) documentation
