Job description

Registered Nurse Case Manager

Are you an RN licensed in Washington, D.C. with an interest to go into Case Management? We are hiring for an RN Case Manager to work with a leading healthcare organization in the DMV! Details are outlined below, interviews are actively going on, apply today!!

Training Overview
  • Training Schedule:
  • 3 week onboarding phase
  • Learn the documentation
  • Learning about the Medicaid Contractual obligations
  • In office meetings every Tuesday to have someone in person to touch base with and ask questions
  • First 3 months they will be in office a bit more to ask questions for training
  • 30 enrollees to start with for all Case managers - will work up to 100 level 1 acuity patients

Job Description

  • What does the caseload look like?
  • Standard RN's will have a caseload of level 1, 2 and 3 acuity patients which will have a total of 75 patients.
  • RN's that have a more advanced understanding of Case Management will be tasked as specialty Case Managers and will take a caseload of 60 individuals total, that have very complex needs at level 3 acuity.
  • Medicaid patients with Socio-economic struggles as well
  • Will work with enrollees and families to break down barriers
  • Have a standard of 3 care plans or assessments done per week (Which means 3 field visit days a week - in office 2 days to wrap up documentation.)
  • Will get list of enrollees well child visits and it is encouraged for the CM to attend (Help to support and update care plan)
  • Team is set up based on who primary care doctor is so that the CM will be able to create a relationship with the provider and best provide support to the Member.

Required Skills
  • DC RN License
  • Ideally Have 2 years of Case Management experience

Nice to Have Skills

  • 5 years of experience
  • Health plan experience
  • Pediatric experience
  • Experience with Medicaid Members

Schedule

  • M-F 8 am - 5 pm
  • Typically 3 field visits a week (Can schedule these how they would like to)

Pay Rate: $42.00-$46.00
