Job Summary
Our client is seeking a Nurse Practitioner to join a national after-hours care team. This is a fully remote position focused on delivering telephonic and virtual care during evenings, nights, weekends, and holidays, providing timely medical guidance and support to residents and facility staff in senior living and skilled nursing settings. In this role, you will be a key clinical resource, managing acute concerns, reducing unnecessary hospital transfers, and ensuring continuity of care.
Key Responsibilities
- Provide after-hours clinical support to patients and facility staff via phone and telehealth platforms.
- Assess and manage acute, chronic, and behavioral health concerns in collaboration with physicians and care teams.
- Coordinate care across care settings, including communication with facilities, hospitals, and outpatient providers.
- Conduct thorough clinical assessments and document all encounters in the electronic medical record in compliance with regulatory standards.
- Apply established clinical protocols and guidelines to support safe, effective treatment in place when appropriate.
- Participate in ongoing training and education to maintain clinical competency and compliance.
Qualifications
- Active, unrestricted Nurse Practitioner license in at least two states (IA, MO, OH, KS in order of priority, must have at least 2), with willingness to obtain additional licenses as needed.
- Eligibility to obtain prescriptive authority and DEA licensure.
- Ability to work independently and manage multiple patient interactions simultaneously.
- Proficiency with electronic medical records and clinical documentation.
- Experience working in skilled nursing facilities, senior living, or post-acute care settings (Preferred).
- Strong understanding of geriatric care, chronic disease management, and acute condition evaluation (Preferred).
- Experience with advanced illness care and end-of-life discussions (Preferred).
- Strong communication skills and ability to build relationships with interdisciplinary teams (Preferred).
- Adaptability in a fast-paced, evolving care environment (Preferred).
Experience
- Minimum of 3 years of clinical experience as a Nurse Practitioner.
- Experience managing acute and chronic conditions in adult or geriatric populations.
Skills

Education
- Master’s degree as a Nurse Practitioner.

Additional Requirements
- This is a fully remote position.
- Comfort with high call volumes and independent decision-making.
- Approximately 24 hours per week with a consistent, set schedule.
- Evening and overnight weekday shifts are required (5pm - 8am).
- Rotating weekend coverage with extended shifts (Friday 5pm to Monday 8am).
- Saturday and Sunday shifts will typically be broken into 12-hour shifts from 6am-6pm.
- Participation in holiday coverage is a team expectation, including two 12-hour holiday shifts per year.
