Public - Responsibilities
Overview of responsibilities (day-to-day, metrics, etc):
- Manage and confirm medical, dental, therapy, and specialty participant appointments adhering to PACE, CMS, and DHCS regulatory timeframes.
- Schedule external provider visits (specialists, diagnostic centers, hospitals), coordinate referrals/authorizations, and arrange transportation.
- Organize orientation days and initial multi-disciplinary assessments (medical, therapy, social work, nursing, Day Center) for new participants.
- Act as scheduling "quarterback" to minimize conflicts and optimize participant and staff schedules across care settings.
- Serve as primary liaison between Senior Center Manager, Clinic Staff, Medical Records, Home Care, and Transportation Coordinators.
- Place confirmation calls to participants and manage last-minute cancellations or urgent scheduling requests.
- Maintain accurate appointment documentation in the Electronic Medical Record (EMR) system.
Public - Required Skills
- Education: High School Diploma or GED.
- Experience: Minimum 2 years in medical scheduling, healthcare administration, or a related field.
- Technical: Hands-on experience using Electronic Medical Records (EMR).
- Health Screening: Contingent upon proof of Tuberculosis screening upon hire (or negative test within the last year)
Public - Preferred Skills
- Elderly Care: Minimum 1 year working with frail elderly participants.
- Language: Bilingual in English and Spanish.
