Job Summary
Our client is seeking a dedicated Registered Nurse to provide essential community-based patient care. This role involves conducting field visits driven by hospital discharge lists and urgent post-hospital updates, ensuring regulatory checks are completed promptly. The successful candidate will participate in daily clinical huddles, manage a target efficiency of 10 field patient visits per day, and meticulously document patient workflows and clinical updates in the CareHub EHR system.
Key Responsibilities
- Conduct community-based patient visits driven by hospital discharge lists and urgent post-hospital/incident updates. Ensure regulatory checks are completed within required windows (internal target of 24–48 hours).
- Participate in mandatory in-person morning huddles at 8:00 AM to align on the day's clinical priorities (remote/Teams options allowed for early medication visits).
- Manage a target efficiency metric of 10 field patient visits per day once fully oriented.
- Efficiently log patient workflows and clinical updates in the CareHub EHR system.
Qualifications
- Ability to perform blood draws, handle biohazard specimens safely, and practice universal precautions.
- Experience administering oral, topical, injectable, and IV medications/immunizations, as well as filling and managing patient medication planners.
- Ability to conduct fingersticks for blood sugar and INR tracking.
- Experience with wound care, ear lavage, and measuring/applying compression garments.
- Ability to conduct health and home safety assessments and reconcile medications following patient hospitalizations, ER visits, or falls.
- Ability to effectively teach caregivers and patients how to manage medications, inhalers, nebulizer treatments, and home testing equipment.
- Comfort working independently under moderate pressure with frail, disabled, or confused elderly participants who may occasionally exhibit physical or verbal aggression.
- Ability to navigate diverse home environments (including stairs and uneven terrain), bend or kneel comfortably, and lift up to 25 lbs (or push/pull up to 150 lbs with equipment).
Experience
- One year of health care experience in a hospital, long-term care, or home health care setting.
- One year of experience specifically working with a frail or elderly population.
Skills
- Proficiency in using Electronic Health Records (EHR) and utilizing the SBAR format to communicate patient status to medical providers.
Education
- Must be a graduate of an accredited Registered Nurse program with an active Colorado RN License.
Additional Requirements
- Valid driver’s license, reliable personal transportation, and active automobile insurance for traveling to patient homes.
- Training will be Monday - Friday 8:00 AM - 5:00 PM for 4 weeks.
- After training, flexible schedules are available: Sunday–Wednesday or Wednesday–Saturday 8:00 AM - 6:30 PM.
- No on-call requirements.
