Nurse Practitioner (NP) / Physician Assistant (PA) – Mobile Wound Care (Eugene)
Position Summary
A growing, employee-focused healthcare team is seeking a dedicated, patient-focused Nurse Practitioner (NP) or Physician Assistant (PA) in Eugene. In this home- and facility-based mobile role, you will evaluate, diagnose, and manage complex wound care and skin conditions for patients in skilled nursing facilities, adult family homes, and private residences.
This role offers significant clinical autonomy, a supportive team structure ("pods"), comprehensive training for new graduates, and company-sponsored certification to help you advance your practice.
Key Responsibilities

Clinical Care & Assessment: Deliver mobile wound care management, evaluating and treating skin conditions and complex wounds across assigned territories.

Treatment Planning: Develop, implement, and monitor evidence-based wound care treatment plans with a strong focus on prevention, healing, and long-term maintenance.

Interdisciplinary Collaboration: Work closely with primary care providers, facility directors, and nursing staff to ensure integrated patient care.

Staff Education & Support: Educate facility staff on best practices, wound management techniques, and prevention protocols to foster buy-in and enhance care outcomes.

Autonomous Panel Management: Independently manage patient scheduling and clinical workflow while collaborating with your assigned provider pod.

Documentation & Compliance: Complete timely, accurate clinical charting and documentation within required electronic medical record (EMR) timelines.

Territory Travel: Travel between patient locations and facilities across the designated territory.
Qualifications & Requirements

Licensure: Active and unencumbered license as a Nurse Practitioner (NP) or Physician Assistant (PA).

Experience Level: Open to both experienced providers and motivated New Graduates.

Travel: Valid driver’s license and willingness/ability to travel regularly within the assigned territory.

Certifications: Certified Wound Specialist (CWS) or equivalent wound care certification is preferred (or a strong interest in obtaining certification with company support).

Key Traits: High accountability, self-motivated, strong interpersonal communication skills, and comfort working independently in diverse care environments.
Schedule & Work Environment

Schedule: Full-time, Monday – Friday (Approximately 6:00 AM – 3:00 PM / 3:30 PM).

Work Style: Daytime mobile hours; flexible schedule based on patient needs without a rigid punch-in clock.

Setting: Mobile / Field-Based (Skilled Nursing Facilities, Adult Family Homes, Patient Homes).
Compensation & Benefits

Base Salary Range: $130,000 – $175,000 / year (Target starting range: $130,000 – $152,000+, depending on licensure, background, and experience).

Incentive Compensation: Monthly individual productivity bonuses ($300 – $6,000+ potential) plus quarterly pod performance bonuses.

Education & Credentials: $3,000 annual CME allowance, company-covered license renewals, additional state license funding, and full sponsorship for CWS certification.
