Job Summary
Our client is seeking a dedicated Nurse Practitioner to provide mobile wound care to patients in various settings, including skilled nursing facilities, patients’ homes, and adult family homes. This role involves evaluating and treating complex wounds and skin conditions, focusing on both prevention and healing, while collaborating with primary care providers and facility staff. The position offers significant autonomy within a provider pod/team structure.
Key Responsibilities
- Provide mobile wound care to patients in skilled nursing facilities, patients’ homes, and adult family homes.
- Evaluate and treat wounds and skin conditions, including complex wounds.
- Develop and recommend appropriate wound-care treatments and interventions.
- Focus on wound prevention as well as healing and maintenance.
- Collaborate with the patient’s primary care provider and facility staff.
- Educate and work with facility staff, including ongoing training/re-training when needed.
- Build relationships and gain staff buy-in to improve patient outcomes.
- Operate with significant autonomy over their patient panel while collaborating with their provider pod/team.
- Travel between patient locations throughout the assigned territory.
- Document patient care and close charts within expected timelines.
Qualifications
- New graduates are welcome.
- Wound care certification is preferred, and the company supports providers in obtaining CWS (Certified Wound Specialist) certification.
Education
- Nurse Practitioner (NP) or Physician Assistant (PA) qualification.
Additional Requirements
- Willingness and ability to travel throughout the assigned territory.
