Job Summary
Our client is seeking a skilled Home Health Clinical Referral Coordinator responsible for managing and processing referrals for home health services. This role serves as the primary point of contact in the referral process, ensuring that all necessary documentation and information is collected for the initiation or resumption of services. The position demands clinical expertise, technical skills, and a commitment to quality, accuracy, and timeliness.
Key Responsibilities
- Reviewing, processing, and verifying all referral elements for admission.
- Coordinating referral management with sources and internal teammates.
- Maintaining consistent knowledge of program referral capacity and managing targets.
- Collaborating with teammates, referral sources, and patients/families on care processes.
- Driving clinical decisions in collaboration with program teammates.
- Supporting teammates by teaching, coaching, and guiding them.
- Engaging with referral portals for effective management.
- Participating in agency quality improvement programs as needed.
- Applying policies and procedures of the home health program.
- Maintaining confidentiality of necessary information.
- Utilizing best practices to drive efficient processes.
- Managing patient records and updating systems.
- Attending training courses/sessions as required.
- Preparing reports and data as needed.
- Organizing and prioritizing large volumes of information.
Qualifications
- High school diploma or GED required.
- Two to three years of related home health referral/intake experience.
- Associate or Bachelor's degree preferred.
- Experience in health insurance verification, prior authorizations, referrals, and Medicare/Medicaid billing preferred.
