Job Summary
Our client is seeking a dedicated and entrepreneurial Care Navigator to own the care experience for a panel of senior patients and their families. This role involves developing individualized care plans, coordinating care across various healthcare partners, and managing post-hospitalization follow-ups. The Care Navigator will also play a pivotal role in building out the care model by developing repeatable workflows and processes, and will eventually contribute to building, leading, and overseeing a growing care team, serving as a clinical leader and patient advocate.
Key Responsibilities
- Own the care experience for a growing panel of senior patients and their families.
- Develop thoughtful, individualized care plans based on each patient's health, goals, and circumstances.
- Coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other healthcare partners.
- Manage post-hospitalization follow-ups and ongoing care needs.
- Coordinate referrals, appointments, and other necessary services.
- Support medication reconciliation and identify potential gaps or issues across providers and pharmacies.
- Help patients and families navigate insurance issues, including denials, prior authorizations, and appeals.
- Maintain consistent communication with patients and family members.
- Attend or support appointments when appropriate.
- Ensure important follow-up items and action plans are completed.
- Serve as a consistent and trusted resource for patients and their families.
- Independently manage an initial patient population while maintaining a high standard of care.
- Identify recurring challenges and opportunities within the patient experience.
- Help develop repeatable workflows, processes, scripts, escalation protocols, and handoffs.
- Document best practices and lessons learned from working directly with patients.
- Partner with technology and product stakeholders to identify ways to improve care delivery.
- Provide a clinical perspective on the development of tools, workflows, and technology.
- Help establish the foundation for a scalable care-delivery model.
- Help define the skills, backgrounds, and characteristics needed for future clinical team members.
- Participate in recruiting and onboarding future Care Navigators.
- Train and mentor new team members using the processes and best practices you help establish.
- Help establish quality standards for patient care.
- Eventually take responsibility for building, leading, and overseeing a growing care team.
- Help ensure patients receive a consistent, high-quality experience as the organization scales.
- Serve as a clinical leader and advocate for the patient as the organization grows.
Qualifications
- Active RN/NP/PA licensure.
- Demonstrated ability to work independently and take full ownership of a patient population with minimal day-to-day direction.
- Strong clinical judgment and the ability to assess complex situations, prioritize needs, and develop thoughtful, individualized care plans.
- Comfortable operating in an environment where processes, workflows, and best practices are still being built (startup mindset).
- Eager to learn, seek feedback, experiment, identify opportunities for improvement, and continuously raise the standard of care (growth mindset).

Education
- Active RN/NP/PA licensure is required.
Additional Requirements
- Willingness to work five days per week onsite. (Patient interactions will be virtual/over phone).
