Job Title: Nurse Practitioner - Home-Based Primary Care

Job Summary Medix is partnering with a premier healthcare provider to recruit a compassionate, dependable, and highly skilled Nurse Practitioner to join a growing team. In this role, you will deliver high-quality primary care services directly to patients within their homes and assisted living facilities. Supported by a highly collaborative clinical leadership team, you will manage a patient panel with a focus on comprehensive care, chronic disease management, and minimizing unnecessary hospital visits.

Pay Rate $60.00 - $72.00 per hour (Equivalent to $125,000 - $150,000 annual base compensation, dependent on experience)

Responsibilities

• Conduct home-based primary care visits in private residences and assisted living facilities, seeing approximately 6 to 10 patients per day.

• Perform varied appointment types including follow-up visits (30 minutes), post-hospitalization evaluations (45 minutes), and comprehensive new patient assessments (60 minutes).

• Manage chronic conditions and deliver personalized primary care to help improve patient outcomes and reduce acute hospital visits.

• Practice independently or alongside a Medical Assistant in the field, utilizing efficient daily travel schedules optimized by local scheduling teams.

• Utilize Athena EMR for documentation, patient management, and communication.

• Participate in weekly team collaboration sessions, including staff meetings, high-risk patient huddles, and panel management reviews.

• Maintain flexibility to adapt to field conditions while adhering to safety protocols and infection control standards.

Schedule

• Monday through Friday, 8:30 a.m. – 5:00 p.m.

• Full-time schedule with structured patient contact and travel built directly into the workday.

• 4-day workweek flexibility options available.

Required Qualifications

• Active Nurse Practitioner (NP) license in the state of Virginia.

• Active DEA license (or eligibility to obtain prior to start).

• Minimum of 3 years of clinical experience in primary care.

• Strong background in Chronic Disease Management.

• Valid driver’s license with an acceptable driving record.

• Comfort and willingness to conduct home visits across diverse residential environments.

Preferred Qualifications

• Prior experience working with geriatric populations, home health, or hospice care.

• Hands-on experience with Athena EMR software.
