Job Summary

Our client is seeking an Assessor to play a critical role in a state-mandated initiative aimed at assisting functionally and chronically disabled adults and children. The goal is to help them remain independent and avoid institutionalization. As an Assessor, you will become a crucial guide and advocate, influencing how clients interact with support systems.

Key Responsibilities

• Conduct comprehensive assessments including analyzing medical and psychosocial information, home visits, and client needs evaluation to identify appropriate services.
• Develop personalized care plans to help clients access home and community-based services for maintaining independence.
• Coordinate and connect by referring clients to resources, consulting with professionals, and ensuring a seamless support system.
• Advocate for client autonomy by promoting self-determination and fair access to vital services.
• Maintain accurate records to track progress, inform service decisions, and contribute to the improvement of the program.

Qualifications

• Holds a valid State of Maryland RN license.
• 2 years of experience in med-surg or case management.
• Reliable means of transportation and a valid driver's license.
• Preferred: Previous experience in home assessment and/or case management.

Experience

• 2 years of experience in med-surg or case management.

Skills

• Assessment and case management skills.
• Ability to conduct medical and psychosocial information analysis.
• Strong advocacy and communication skills.

Additional Requirements

• 30-40 hours weekly, Monday to Friday. Open to a minimum of 3 days per week with a set schedule and completion of a minimum of 4 assessments per day. Assessments scheduled according to geographical area and coverage.
