Job Summary

Our client is seeking a Care Coordinator to join a dynamic team dedicated to delivering comprehensive healthcare services. The primary responsibilities of this role include collaborating with an interdisciplinary team, managing referrals, maintaining accurate documentation, and fostering relationships with members to ensure seamless care coordination.

Key Responsibilities

• Collaborate with an interdisciplinary team to understand members' care needs.
• Coordinate referrals to external providers and services.
• Maintain accurate documentation for member referrals.
• Build and nurture strong relationships with members and their families.
• Monitor care plans to ensure follow-up and progression on care goals.
• Adhere to HIPAA standards and confidentiality guidelines.
• Facilitate communication among care teams and clinicians.
• Resolve barriers to care, including navigating the healthcare system and financial constraints.
• Act as the primary system navigator and contact for members and families.
• Perform additional duties as assigned.

Qualifications

• Deep understanding of the healthcare ecosystem, with experience in benefits and insurance authorization.
• Genuine passion for helping members carry out their care plans successfully.
• Effective collaboration with diverse populations including clinic staff, providers, and insurance carriers.
• Proficiency in basic medical terminology, CPT, and ICD-10 codes.
• Understanding of Medicare criteria for medical equipment and supplies.
• Strong knowledge of specialists and community resources.
• Exceptional communication skills and ability to build rapport in an interdisciplinary environment.
• Proven multitasking abilities and attention to detail.
• Proficient computer skills, including Microsoft Office suite.

Experience

• Minimum of 2 years of experience in referral management or healthcare coordination.
• Background as a Medical Assistant (MA) or in medical administrative roles is highly beneficial.
• Experience using Electronic Medical Records (EMR) systems like eCW or similar platforms.
