Job Summary
Our client is seeking a Telephonic Case Manager to provide telephonic case management to medically complex and chronically ill members. Responsibilities include conducting comprehensive health assessments, creating individualized care plans, coordinating care with partners, and providing clinical/disease state education.
Responsibilities / Job Duties
- Provide telephonic case management to medically complex and chronically ill members.
- Conduct comprehensive health assessments and create individualized care plans.
- Coordinate care with internal and external partners.
- Conduct ICT/IDT meetings presenting members to nurse practitioners/RMOs and social workers.
- Provide clinical/disease state education on topics such as CHF, A1C, medications, and labs.
- Identify gaps in care and resolve service/access issues.
- Make approximately 30–40 calls daily, spending 3–4 hours on the phone.
- Document directly in AVA care plan system.
- Delegate non-clinical coordination tasks to assigned Care Coordinators.
Minimum Education and Experience Qualification Requirements
Education
- Bachelor's Degree in Nursing (BSN)
- RN License
Qualifications
- Fully bilingual/fluent in Spanish.
- 1 Year of Case Management Experience.
- 2+ years clinical RN exp
- Familiarity with Medicare Advantage / Medicaid vs. Medicare.
- Must have strong verbal communication skills.
- Self-motivated with good computer skills, including self note-taking.
Schedule / Shift
8:00am - 5:00pm, Monday-Friday, PST with a one-hour lunch break. Fully Remote. Must have an RN license in California and a HIPAA compliant work space. May work from anywhere in the U.S., but must work Pacific Time hours.
