Job Summary

Our client is seeking a Nurse Care Manager responsible for overseeing chronic care and transitions of care management of high-risk patients. This includes coordinating patient care, facilitating transitions from acute to home care, and collaborating with multidisciplinary teams to ensure efficient and high-quality care delivery.

Key Responsibilities

• Oversee chronic care and transitions of care management of high-risk patients within care teams and neighborhood centers.
• Serve as a resource for the multidisciplinary team for the management of complex patients, including assessments and care plans.
• Perform triage for patients via phone and address issues appropriately.
• Ensure efficient, organized patient transitions from acute and post-acute settings.
• Conduct comprehensive assessments for physical, mental, and social risk factors.
• Collaborate with medical staff to eliminate barriers to care delivery.
• Coordinate patient care progression throughout the continuum.
• Facilitate care for designated patients and monitor their progress, intervening as necessary.
• Coordinate post-discharge needs and track readmissions to acute care facilities.
• Utilize advanced conflict resolution skills to ensure timely resolution of issues.
• Participate in clinical performance improvement activities and document interactions in the Electronic Medical Record.

Qualifications

• Bachelor’s degree in Nursing.
• Active Arizona Registered Nurse License.
• Bilingual in English and Spanish.
• Minimum 2 years of experience as a Registered Nurse.
• Minimum 2 years of experience in utilization management, case management, or related fields.
• Current working knowledge of chronic care management, discharge planning, and managed care reimbursement.
• Experience with Medicare Advantage, Value-based care, and/or Managed Care is desirable.
