Job Summary

Our client is seeking a dedicated Nurse Care Manager responsible for overseeing chronic care and transitions of care management for high-risk patients. The role involves working as a resource for the multidisciplinary team, coordinating patient care, and ensuring efficient care delivery and patient transition management.

Key Responsibilities

• Manage chronic care and transition of care for high-risk patients within care teams and neighborhood centers.
• Serve as a resource for the multidisciplinary team on complex patient management, including assessments and care plans.
• Perform patient triage via phone and manage care transitions from acute/post-acute settings to home or transitional facilities.
• Conduct comprehensive assessments of physical, mental, and social risk factors, identifying and addressing barriers.
• Coordinate and facilitate patient care progression and work collaboratively to identify and resolve barriers to efficient care.
• Facilitate diagnostics, treatment plans, and discharges on a timely basis, ensuring a focus on patient-centered, high-quality, and cost-effective care.
• Coordinate patient discharge plans ensuring readiness and participation from all care team members.
• Utilize the Coleman Model and coordinate post-discharge needs such as medical equipment and medications.
• Proactively identify and resolve diagnostic, treatment, and discharge issues to help prevent readmissions and hospitalizations.
• Collect, analyze, and address variances in the plan of care, and participate actively in clinical performance improvement activities.

Qualifications

• Bilingual/Bicultural (English and Spanish).
• Minimum 2 years of experience as a Registered Nurse (Active license needed).
• Minimum 2 years of experience in utilization management, case management, or related fields.
• Available to work during assigned clinic business hours.
• Competency in chronic care management, discharge planning, and managed care reimbursement.
• Experience with Medicare Advantage, Value-Based Care, and/or Managed Care is desirable.

Additional Requirements

• Hybrid work environment: 2-3 days at the clinic, other days remote.
