Job Summary

Our client is seeking a dedicated Registered Nurse to coordinate, triage, and manage clinical care for Veterans receiving medical services through non-VA community healthcare providers. This role acts as a clinical bridge between the VA, the Veteran, and community facilities, ensuring Veterans receive timely and high-quality care that aligns with VA guidelines and community care pathways.

Key Responsibilities

• Clinical Triage & Referral Review: Review and assess clinical consults for community care appropriateness. Triage incoming requests for urgent vs. routine care and determine the appropriate level of care within the scope of nursing practice.
• Community Care Coordination: Serve as a clinical liaison between VA health teams, Veterans, and external non-VA specialists, diagnostic centers, and community hospitals. Communicate care plans and clinical expectations to external community providers.
• Medical Record & Results Review: Review clinical documentation, imaging reports, lab results, and discharge summaries submitted by non-VA providers. Integrate external records into the VA Electronic Health Record (EHR) system to ensure continuity of care.
• Medication & Treatment Oversight: Review community provider treatment orders, prescriptions, and recommended follow-up interventions. Coordinate with internal VA pharmacy and primary care teams to ensure safe medication reconciliation.
• Patient Advocacy & Consultation: Perform telephone consultations with Veterans regarding their community care appointments, treatment plans, and post-procedure updates. Assess patient status and instruct Veterans/families on prescribed care regimens.
• Quality Assurance & Safety: Follow all established infection control and patient safety standards. Document all patient encounters and procedure notes in the EHR, report safety incidents within 24 hours via the JPSR system, and participate in peer review/quality improvement processes.

Qualifications

• Graduate of a state-approved professional nursing school accredited by ACEN or CCNE.
• Active, unrestricted RN license from the Ohio Board of Nursing (OBN).

Experience

• Minimum of 1 year of direct patient care or clinical care management experience within the last 3 years.

Skills

• Technical skills: Must hold active CPR (AHA) and BLS certifications.
• Soft skills: Effective communication and coordination skills.

Additional Requirements

• Tier 1 / Low Risk background investigation clearance, PIV card issuance, and complete immunization/TB clearance.
• Annual completion of required VA training (EHR, VA Privacy/HIPAA, Cyber Security, Active Threat, Prevention of Workplace Harassment).
