Position Overview
We are seeking an empathetic, detail-oriented, and organized Care Coordinator / Referral Specialist to join a rapidly growing, senior-focused healthcare provider. This role serves as the critical bridge between senior members, internal clinical teams, and external specialists to ensure seamless, high-quality care.
This position is primarily administrative and phone-based (~60% member outreach, ~40% provider communication), with occasional face-to-face member interaction at the clinic. It is ideal for a proactive professional who thrives on solving logistical barriers, building rapport with elderly patients and their families, and managing complex referral pipelines.
Key Responsibilities

Specialist & Outreach Coordination: Manage high-volume outbound and inbound calls to coordinate specialist appointments (e.g., Cardiology, Oncology) and remind members of preventive care screenings.

Patient Navigation & Support: Act as a primary point of contact for members and families, addressing care plan questions, addressing transportation or language barriers, and guiding them through the healthcare system.

Interdisciplinary Collaboration: Partner closely with Nurse Care Managers, Social Workers, and clinic staff to close care gaps and track post-appointment feedback/notes from external provider offices.

Referral Management & Documentation: Obtain insurance authorizations, submit external referrals, and maintain accurate EMR records in compliance with HIPAA guidelines.

Care Plan Monitoring: Monitor ongoing progress on care goals, ensuring timely routine follow-up for medical equipment, supplies, and external consultations.
Qualifications & Skills
Required:

Experience: Minimum 2 years of direct referral management, healthcare coordination, or surgery scheduling experience.

Technical Knowledge: Proficiency in basic medical terminology, CPT/ICD-10 coding, EMR platforms (e.g., eCW), and Microsoft Office.

Medicare & Insurance: Familiarity with Medicare criteria for medical supplies/equipment and insurance authorization workflows.

Communication: Exceptional phone etiquette and patient-facing soft skills, with a compassionate, patient approach tailored to an older adult population.

Efficiency: Strong computer skills with the ability to multi-task and manage high call volumes and referral tracking backlogs efficiently.
Preferred:

Clinical/Admin Background: Prior experience as a Medical Assistant (MA), Licensed Practical Nurse (LPN), or Senior Administrative Coordinator in a primary care or specialty clinic.

Education: High School Diploma required; Associate degree or coursework in healthcare management/allied health preferred.

Bilingual Skills: Fluency in Spanish or other community-spoken languages is a plus.
