Clinical Quality LVN
Location: San Antonio, TX (Onsite)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM CST
Pay Rate: $28.00 – $30.00/hr (Depending on Experience)
Position Summary
We are seeking a detail-oriented Licensed Vocational Nurse (LVN) to join our Quality team in San Antonio. In this role, you will focus on closing HEDIS/STAR care gaps by conducting high-volume outbound calls to health plan members. You will educate patients on missing preventive services, assist with appointment scheduling, address medication non-compliance, and accurately document interactions to drive better healthcare outcomes.
Key Responsibilities

Make high-volume telephonic outreach (15–20 minutes per call) using a dialer system or manual lists.

Identify members with missing quality measures and work to close gaps through education, counseling, and scheduling assistance.

Assist members with inbound inquiries or route them to appropriate departments as needed.

Document member and provider records accurately in the system while adhering to established system scripting.

Support local leadership and clinics with quality data collection, performance reporting, and gap closure strategies.

Maintain an up-to-date knowledge base of HEDIS and STAR standards.
Qualifications

Licensure: Active Texas LVN license required.

Experience: Minimum 3+ years of healthcare experience, including managed care settings.

Technical Skills: 3+ years of experience with data analysis or quality chart reviews, along with proficiency in Microsoft Office (Word, Excel, databases).

Communication: Exceptional phone presence and soft skills; prior experience in a call center, phone-based triage, or outpatient setting preferred.

Adaptability: Ability to navigate multiple software applications simultaneously while documenting live calls.
Preferred: Experience with HEDIS/STAR measures and 2+ years of college coursework toward an Associate's or Bachelor's degree. (Note: Candidates with backgrounds strictly in LTAC or Skilled Nursing Facilities may not be the optimal fit for this high-volume outbound call environment.)
