Job Summary
Our client is seeking a dedicated Nurse Practitioner to deliver comprehensive medical care to patients in their homes. This role involves conducting health assessments, diagnosing and treating health conditions, and creating treatment plans. The Nurse Practitioner will also perform clinical procedures, coordinate follow-up care, and educate patients and caregivers.
Responsibilities / Job Duties
- Deliver comprehensive medical care to patients in their homes.
- Conduct thorough health assessments, evaluations, and physical examinations.
- Diagnose and treat acute and chronic health conditions.
- Develop and implement individualized treatment plans.
- Perform clinical procedures within scope of practice, including EKGs and catheterizations.
- Order, interpret, and communicate diagnostic testing results.
- Coordinate follow-up care and referrals as necessary.
- Educate patients and caregivers regarding treatment plans and ongoing care needs.
- Accurately document patient encounters and ensure compliance with clinical standards.
- Exercise independent clinical decision-making while delivering patient-focused care.
- Collaborate with operational and clinical leadership to ensure quality outcomes and patient satisfaction.
Minimum Education and Experience Qualification Requirements
Education
- Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
- National board certification.
- Current BLS and ACLS certifications.
- DEA registration or willingness to obtain upon hire.
Qualifications
- Minimum of three years of advanced practice provider experience in Emergency Medicine, Urgent Care, Hospital-Based Practice, or Emergency Department.
- Demonstrated hands-on procedural experience, including EKGs and catheterizations.
- Valid driver's license and reliable transportation.
- Ability to work independently in a home-based care environment.

Schedule / Shift
40-hour work week consisting of four 10-hour shifts. Shift times typically operate from 8:00 AM to 6:00 PM or 10:00 AM to 8:00 PM. Includes 4 weekend days per month, with options for one Saturday or Sunday per week. Extra shift pay available for additional shifts. Weekends not required for the first 30 days; incentives available for weekend shifts.
