Job Summary

Our client is seeking a Licensed Vocational Nurse to conduct focused data collection and basic assessments for assigned member cases. The role involves reviewing clinical records, drafting individual care plan components, and preparing SBAR summaries under the guidance of an RN. The position requires maintaining accurate documentation in accordance with SNP requirements and supporting care coordination outreach tasks.

Responsibilities / Job Duties

• Conduct focused data collection and basic assessments for assigned member cases.
• Review clinical records, medication lists, encounter history, and care plan status.
• Draft ICP components based on outreach and member needs, under RN guidance.
• Prepare SBAR summaries for RN review and finalization.
• Report any significant changes or red-flag findings to the supervising RN.
• Maintain accurate, timely documentation in accordance with SNP requirements.
• Support care coordination outreach and follow-up tasks within LVN scope.
• Productivity monitored daily.

Minimum Education and Experience Qualification Requirements

Education

• Current California LVN license in good standing.

Qualifications

• At least 1 year of experience.
• At least 1 year of experience in care coordination, ambulatory care, or managed care preferred.
• Experience with chart review and care plan implementation preferred.
• Strong attention to detail and ability to follow workflows.
• Excellent documentation accuracy, and communication skills.

Schedule / Shift

Monday - Friday: 8am or 8:30am - 5pm or 5:30pm PST. Fully remote throughout California. 40 hours per week.
