Job Summary

We are seeking a reliable, detail-oriented, and compassionate Licensed Practical Nurse (LPN) to join our Medication Assisted Treatment (MAT) clinical team. In this role, you will play a key part in delivering specialized care to individuals recovering from substance use disorders. You will be responsible for accurately dispensing medications (Methadone, Buprenorphine), maintaining strict inventory and DEA diversion controls, and utilizing ZooBook EHR to ensure compliant real-time documentation.

Shift & Schedule

• Hours: Part-time, 20 to 29 hours per week

• Monday – Friday: 6:30 a.m. – 3:00 p.m. (Note: Dosing window closes daily between 10:30 a.m. and 12:30 p.m.)

• Saturday: 8:30 a.m. – 1:00 p.m. (Shift departure by 1:30–2:00 p.m.)

• Sundays: Closed

Key Responsibilities

• Medication Administration & Safety: Accurately dispense and administer Methadone, Buprenorphine, and other MAT medications strictly following physician orders. Enforce mandatory minimum two-patient identifier protocols prior to administering every dose.

• Patient Assessment: Monitor patients at the dosing window for signs of intoxication, withdrawal, sedation, or adverse reactions. Exercise clinical judgment to hold doses and immediately escalate concerns to medical staff when indicated.

• Dispensing System & Calibration: Prepare daily cassettes and bottle sets; perform daily calibration checks and routine troubleshooting of automated medication dispensing equipment.

• Controlled Substance Reconciliation: Conduct daily narcotic counts and reconcile controlled substance inventory in accordance with DEA, state, and CARF regulatory standards. Report any inventory discrepancies to the Nurse Supervisor immediately.

• EHR Documentation: Maintain real-time documentation in ZooBook EHR, recording dosing, take-home bottle counts, missed doses, point-of-care testing (Urine Drug Screens, Breathalyzer), and adverse reactions.

• Take-Home Medication Protocols: Administer and record take-home medications in compliance with federal take-home eligibility criteria and individual treatment plan phases.

• Facility Security & Safety: Maintain security of the medication room, safe, and controlled substances at all times by adhering to strict double-lock and dual-custody procedures. Operate behind a protected glass dosing window in a building equipped with dedicated on-site armed security.

• Interdisciplinary Coordination: Collaborate closely with counseling, medical staff, and supervisors regarding patient phase advancements, status changes, and treatment plan updates.

• Audit Preparedness & Compliance: Maintain audit-ready dispensing logs and records for federal (DEA, 42 CFR Part 8), state (IDHS/DBHR), and CARF surveys.

• Emergency Response: Respond promptly to medical emergencies, including administering Naloxone and initiating emergency protocols when required.

Requirements & Qualifications

• License: Current, unrestricted Illinois LPN License in good standing.

• Certification: Current CPR / BLS certification.

• Experience: Minimum 1 year of professional nursing experience. Prior experience in an Opioid Treatment Program (OTP), recovery clinic, or substance use disorder setting is strongly preferred.

• Technical Experience: Experience using Electronic Health Records (ZooBook preferred) and electronic medication dispensing equipment.

• Regulatory Knowledge: Working knowledge of (or willingness to be trained on) 42 CFR Part 2 & Part 8, DEA diversion-control procedures, and state OTP standards.

• Background Check: Ability to successfully pass required state/federal background checks, drug screenings, and fingerprinting per OTP mandates.

• Key Attributes: Excellent de-escalation skills, strong attention to detail, highly punctual, self-directed, and committed to a non-judgmental, compassionate approach to patient care.

Physical Demands

• Ability to stand, bend, and reach for extended periods during dosing window hours.

• Manual dexterity required for operating pump equipment and preparing take-home medication bottles.

• Ability to lift up to 25 lbs (medication stock and supply totes).
