Job Summary
Our client is seeking a Registered Nurse Case Manager. The primary responsibilities include conducting comprehensive clinical assessments, maintaining case files, developing care plans, coordinating member care, facilitating transitions of care, and conducting telephonic assessments. This role ensures that members receive personalized care and support to achieve functional independence in their community.
Responsibilities / Job Duties
- Conduct, review, and document comprehensive clinical assessments and follow-up interventions.
- Maintain case files ensuring documentation meets policy, state, and federal requirements.
- Develop and implement member-specific care plans with prioritized goals.
- Coordinate member care with various health providers to support functional independence.
- Facilitate transitions of care between settings and coordinate necessary services.
- Support long-term care members transitioning to lower care levels or community settings.
- Conduct telephonic assessments.
Minimum Education and Experience Qualification Requirements
Qualifications
- Active California RN License without restriction.
- Minimum 3 years of experience in case management, discharge planning, or equivalent experience.
- Knowledge of managed care principles, emphasis on Utilization and Case Management.
- Clinical knowledge and critical thinking skills for effective care plan development.
- Proficient in applicable computer software including Outlook, Word, Excel, and case management programs.
Skills
- Bi-lingual skills in Spanish, Vietnamese, Chinese, or Tagalog (preferred).
- Experience with behavioral health, seniors, persons with disabilities, or children populations (preferred).
Schedule / Shift
Onsite/Hybrid work hours: 8:30 AM - 5:00 PM, Monday through Friday. Hybrid schedule includes three days onsite and two days remote.
