Job Summary
Our client is seeking a dedicated Nurse Practitioner to provide standard primary care in facilities such as assisted living, independent living, and residential care. The role involves delivering high-quality patient care and ensuring effective management of up to 8 private home patients.
Key Responsibilities
- Provide primary care rounding in various facilities including assisted/independent living and residential settings.
- Manage and care for up to 8 private home patients per Nurse Practitioner.
- Handle 8 to 12 patients per day, with a target of approximately 10 patients per day.
- Perform various medical procedures including vitals checks, physical examinations, wound care, ear cleaning, stitch removal, and administering injections.
Qualifications
- 2–3 years of experience as a Nurse Practitioner, with preference for experience in primary care, home health, or geriatric settings.
- Must possess a Massachusetts NP License.
- Willingness to obtain licensure in New Hampshire, with reimbursement offered after 90 days of employment.
Skills
- Experience with Athena EMR or Spruce communication platform.
- Strong background or skills in social work.
- Excellent communication and interpersonal skills.
- Ability to work independently with a high degree of autonomy.
Additional Requirements
This is a full-time, hybrid role requiring facility rounds in the morning and remote note-taking/admin tasks in the afternoon. The position includes one admin day per week.
