Job Summary
Our client is seeking a Community Growth & Patient Relations Manager for a leading value-based primary care organization. The role involves driving growth by being the face of the practice in the community, building trust with seniors and healthcare partners, and ensuring Medicare-eligible patients receive high-quality, holistic care.
Key Responsibilities
- Drive new patient growth through DTC, B2B, and community stakeholder relationships.
- Advocate the healthcare mission at local gatherings such as senior centers and faith-based organizations.
- Plan and execute local community events targeting seniors.
- Cultivate relationships with health plan agents and community providers.
- Collaborate with the Community Relations team to enhance brand awareness.
- Coordinate with health plan agents for patient enrollment and assignment to clinics.
- Utilize CRM tools to manage patient pipeline and documentation.
- Develop and execute market penetration plans and maintain an activity calendar.
- Engage with new patients to ensure retention and resolve service-related concerns.
Qualifications
- High local mobility to meet patients; limited travel outside the local market.
- Proven track record in community outreach, healthcare sales, or Medicare Advantage markets preferred.
Skills
- Prior sales experience.
- Able to learn Salesforce.
- Driving Insurance and own vehicle required.
- Experience in Medicare sales preferred.
