Position Summary:
The Care Navigator is an ideal position for a proactive, self-motivated, positive individual who will work closely with an inter-professional care team to coordinate patient care and improve patient outcomes. Acting as the day-to-day primary engagement point for patients, the Care Navigator focuses on building and maintaining relationships with patients, community organizations, and care team members. This remote role helps patients utilize available resources while handling daily activities such as appointment scheduling, problem-solving, and patient education.
Note: This is a 100% remote position, but candidates must reside in Arizona and be available for occasional quarterly travel to Phoenix for team training and collaborative sessions.
Duties and Responsibilities

Collaborate with the core functional care team to provide psychosocial interventions and case management services to a designated panel of patients.

Provide indirect care support to medical providers through scheduling, documentation, and referrals.

Manage referrals to specialists, labs, durable medical equipment (DME), and supplemental benefits.

Interact with patients virtually via teleconference or telephone.

Maintain clear channels of communication between care team members, patients, caregivers, and external health professionals.

Organize and update medical records to ensure timely access to patient information.

Answer incoming patient phone calls, troubleshoot issues, and involve care team members as necessary.

Assist with the development and implementation of comprehensive care plans for the patient panel.

Expand knowledge of community resources and guide patients through the procedures required to access them.

Support interprofessional team processes, including gathering patient information and educating patients on the proper use of equipment and resources.

Document all encounters accurately and in a timely manner.

Attend quarterly in-person team meetings and training sessions in the Phoenix metro area.

Maintain strict confidentiality regarding all sensitive patient information.

Support continuous quality improvement efforts and adhere to all organization policies and procedures.
Requirements

Must be a current resident of Arizona.

At least three years of experience and/or education in health or social services. Medical Assistant (MA) certification is a required.

Ability and willingness to travel to Phoenix, AZ on a quarterly basis for required team trainings and meetings.

Must be self-motivated, highly organized, and capable of performing work independently from a dedicated home office.

Ability to develop and maintain inclusive, equitable relationships with patients, caregivers, and staff.

Experience connecting patients with community resources from diverse backgrounds.

Advanced customer service skills, including excellent oral and written communication capabilities.

Comfort and proficiency in interacting with patients and care teams virtually and telephonically.
