Pharmacy Technician – Reconciliation
Position Summary
We are seeking a detail-oriented and patient-focused Pharmacy Technician to join our remote clinical pharmacy support team. This role will support health plan-sponsored pharmacy and medication management programs by engaging directly with patients, providers, pharmacies, and healthcare teams.
The ideal candidate will have strong communication and organizational skills, a solid understanding of medications and pharmacy processes, and experience supporting patients in a clinical, pharmacy, health plan, or healthcare environment.
Key Responsibilities
- Connect with patients by phone to provide support for health plan-sponsored pharmacy and medication management programs.
- Gather relevant patient and provider information to support clinical services and interventions.
- Identify medication- or health-related concerns and appropriately escalate clinical matters to pharmacists.
- Coordinate with patients, providers, pharmacies, hospitals, and clinics to obtain information and resolve outstanding needs.
- Respond to inbound calls and inquiries from patients, providers, and pharmacy teams in a professional, timely, and compassionate manner.
- Maintain accurate and detailed documentation within internal pharmacy, EMR, and clinical systems.
- Schedule patients for telephone and virtual appointments with clinical pharmacists.
- Assist with referrals, authorizations, and related administrative processes as needed.
- Support medication management, medication history, reconciliation, and adherence-related activities as applicable.
- Participate in team meetings, quality initiatives, and workflow improvement projects.
- Support additional pharmacy operations and special projects as assigned.
Required Qualifications
- Active state Pharmacy Technician registration or license, as applicable.
- National Pharmacy Technician certification through PTCB or ExCPT highly preferred.
- Familiarity with common medical terminology and brand/generic medication names.
- Strong organizational skills with the ability to prioritize and manage multiple tasks in a fast-paced environment.
- Excellent written and verbal communication skills.
- Strong computer skills and the ability to work comfortably across multiple technology platforms.
- Ability to work effectively both independently and as part of a collaborative team.
- Availability to work a full-time, 40-hour workweek.
Preferred Experience
- Experience working with electronic medical records (EMR), electronic health records (EHR), or healthcare documentation systems.
- Experience in ambulatory care, medication management, medication therapy management (MTM), medication reconciliation, or medication history collection.
- Experience supporting medication adherence programs or conducting patient outreach.
- Experience working in a high-volume healthcare, pharmacy, health plan, or PBM environment.
- Experience communicating with patients, providers, pharmacies, and clinical teams.
- Bilingual skills are a plus.
Schedule
- Full-time, Monday–Friday
- Operating hours are between 6:00 AM and 6:00 PM Pacific Time.
- Flexibility to work varying shifts within operating hours is preferred.
- Up to two rotating Saturday shifts per month may be required.
