Job Summary
Our client is seeking a Care Management Coordinator who will focus on direct engagement and outreach to individuals based on claim triggers and coverage gaps. The coordinator will assist individuals in navigating their benefits, providing guidance on medical plan coverage, and resolving non-clinical insurance inquiries. Additionally, the role involves care advocacy, case support, interdisciplinary collaboration, and operational reporting.
Key Responsibilities
- Execute proactive outbound communication to individuals based on claim triggers and referral indicators.
- Explain medical plan coverage and guide individuals toward in-network specialists.
- Resolve systemic barriers to care and manage post-discharge follow-ups.
- Collaborate with health coaches to align on care strategies and document transition plans.
- Establish workflows and operational guides scalable across multiple accounts.
Qualifications
- Minimum of 3 years in healthcare navigation/care coordination with a Bachelor’s degree, or 5+ years of relevant healthcare support experience.
- Understanding of health insurance structures and medical terminology.
- Ability to author standard operating procedures (SOPs).
- Experience managing caseloads within CRM systems and using communication tools like MS Teams.
- Strong written communication and problem-solving skills.
- Preferred: Degree or background in Healthcare Management or Social Work.
- Preferred: Certified Case Manager (CCM) or Accredited Case Manager (ACM) credentials.
- Preferred: Experience in fast-paced health management or corporate wellness environments.
Experience
- Minimum of 3 years in healthcare navigation/care coordination with a Bachelor’s degree, or 5+ years of relevant healthcare support experience.
Skills
- Technical proficiency in managing caseloads within CRM systems and using communication tools like MS Teams.
- Strong written communication and active problem-solving skills.
Schedule
Monday thru Friday, 8 am to 5 pm, 40 hours a week.
