Job Summary
Our client is seeking a dedicated Social Worker responsible for delivering clinical social work and complex case management to high-risk patients with intricate psychosocial and financial needs. The role involves collaborating with an interdisciplinary team to evaluate care plans, connect patients to community funding and social programs, and manage resources across the continuum of care, aligning with individual and payer requirements.
Key Responsibilities
- Provides comprehensive care coordination to an assigned patient caseload.
- Works collaboratively to meet the complex medical needs of patients.
- Develops treatment plans through clinical social work diagnosis, assessment, and treatment interventions.
- Intervenes with patients and families regarding the emotional, social, and financial consequences of illness and/or disability.
- Assesses and mobilizes family/community resources to meet social care needs.
- Handles cases involving elder abuse/neglect, domestic violence, guardianship, mental health placement, and sexual assault.
- Assists patients with advance directives and addresses end-of-life decisions.
- Advocates for patient and family empowerment to make autonomous health care decisions.
- Documents all interventions in patient medical records timely and accurately.
Qualifications
- Bilingual in English and Spanish.
- Graduate of an accredited Master of Social Work program.
- Active Arizona state license (LMSW, LCSW, LPC, or LMFT) with 3+ years of social work experience or CHW certification with 6-8 years of experience.
- Knowledge of government/community resources and Medicare/Medicaid services.
Skills
- Experience with assessing and addressing social determinants of health.
- Proficiency in EMRs, computers, mobile devices, medical devices, and Microsoft Office Suite.
- Expertise in connecting patients with community resources and governmental agencies for complex social needs.
