Key Responsibilities

Comprehensive Assessments & Treatment: Evaluate and treat acute and chronic wounds (diabetic ulcers, pressure injuries, vascular/surgical/traumatic wounds, burns, and atypical wounds) across various mobile and facility settings.

Procedures & Advanced Therapies: Perform wound debridement within scope/competency; apply therapies including NPWT, compression systems, skin substitutes, biologics, collagen matrices, and specialty dressings.

Diagnostic & Prescribing Authority: Order/interpret labs, cultures, vascular studies, and imaging; prescribe medications, dressings, supplies, and DME.

Complication Management: Identify and address infection, ischemia, osteomyelitis, and delayed healing.

Care Coordination & Education: Partner with PCPs, podiatrists, vascular specialists, home health agencies, and facility staff; educate patients and caregivers on treatment plans.

Documentation & Logistics: Timely, accurate mobile EMR documentation adhering to HIPAA and infection-control standards; manage assigned routes and clinical supply inventories securely.

Licensure & Certifications
• Active California Nurse Practitioner (NP) license
• National Board Certification
• Active DEA registration (for prescribing)
• Valid CA Driver’s License and reliable vehicle
Clinical Experience
• 1–2 years of dedicated wound-care experience OR completion of advanced wound-care training.
• Proven competency in wound debridement and advanced modalities.
• Experience managing care in mobile, home-based, or facility environments.
Technical & Regulatory
• Proficiency with mobile EMR platforms and point-of-care charting.
• Solid understanding of Medicare-compliant wound documentation and medical necessity.
Preferred Qualifications

Specialty Certifications: WCC, CWCN, CWON, CFNC, or equivalent.

Debridement Proficiency: Hands-on experience with sharp, enzymatic, mechanical, and autolytic debridement.

Payor & Administrative Familiarity: Knowledge of Noridian JE wound-care guidelines and experience participating in clinical audits or QI initiatives.
Schedule & Travel Details

Format: Flexible, field-based schedule. Local travel required across San Jose and nearby coverage zones.

Visits: Pay-per-visit model for completed patient visits; occasional telehealth follow-ups may be integrated as needed. Expected daily visit volumes and specific coverage days will be confirmed during onboarding.
