Job Summary
Our client is looking for a talented Medical Scribe to join their team. The Clinical Informatics Specialist (CIS) collaborates with primary care providers and plays a crucial role within the Care Team. The main focus is on ensuring the accuracy and efficiency of medical documentation during patient encounters. By providing essential support, the CIS enables healthcare professionals to dedicate more time to patient care while ensuring comprehensive and precise patient records are maintained.
Key Responsibilities
- Patient Encounter Documentation: Accompany healthcare providers during patient visits and accurately document all relevant medical information in real-time.
- EHR Management: Utilize electronic health record (EHR) systems to manage patient data, ensuring compliance and confidentiality standards are met.
- Chart Review: Regularly review and organize patient charts, addressing any inconsistencies or discrepancies for completeness and accuracy.
- Effective Communication: Facilitate communication between healthcare providers and team members for exchanging crucial information and follow-up instructions.
- Data Entry and Coding: Enter and manage accurate ICD-10 and CPT codes for diagnoses and services during patient encounters.
- Liaison for Documentation Queries: Serve as a liaison between the coding team and providers for timely resolution of documentation queries.
- Morning Huddle Preparation: Prepare comprehensive research on patients' historical charts to enhance morning huddle presentations.
- Quality Metrics Management: Monitor and report on healthcare quality metrics, identifying gaps in care for optimal outcomes.
- Population Health Optimization: Evaluate patient stratification and enroll at-risk patients in chronic care management protocols.
- Evidence-Based Research: Keep the care team informed with updated evidence-based medical protocols.
- Maintain Confidentiality: Ensure patient data is handled with the utmost discretion, adhering to HIPAA and privacy regulations.
Qualifications
- Bilingual in Spanish and English
Experience
- 2+ years of scribe experience or clinical experience with some scribing.
Skills
- Proficient in electronic health record (EHR) systems.
- Strong communication and teamwork abilities.
