Job Summary
Our client is seeking a dedicated and passionate Care Coordinator to partner with their interdisciplinary care team. The primary goal is to ensure comprehensive care for each member by identifying gaps and barriers in treatment and coordinating referrals to external providers, vendors, and services.
Key Responsibilities
- Collaborate with an Interdisciplinary Team to understand each member's care needs.
- Coordinate Referrals to external providers, vendors, and services.
- Maintain Accurate Documentation for member referrals to ensure records are compliant and current.
- Build strong relationships with members and their families to facilitate care coordination.
- Monitor Care Plans to ensure routine follow-up and achievement of care goals.
- Maintain Compliance with HIPAA standards and confidentiality guidelines.
- Facilitate Communication among the internal care team and external clinicians.
- Resolve Barriers to Care related to healthcare systems, financial constraints, or social barriers.
- Serve as a Patient Navigator, acting as the primary contact for members and families.
- Perform additional duties as assigned.
Qualifications
- Deep understanding of the healthcare ecosystem, ideally with experience in benefits and insurance authorization.
- Genuine passion for helping senior members with their care plans.
- Ability to work effectively with diverse populations.
- Proficiency in basic medical terminology, CPT, and ICD-10 codes.
- Solid understanding of Medicare criteria for medical equipment and supplies.
- Strong knowledge of specialists and community resources.
- Exceptional communication and rapport-building ability.
- Proven ability to multi-task and manage competing priorities.
- Proficient computer skills, including Microsoft Office suite.
Experience
- Minimum of 2 years of experience in referral management or healthcare coordination.
- Medical Assistant (MA) or medical administrative experience is highly helpful.
- Experience utilizing Electronic Medical Records (EMR) systems like eCW.
Education
- H.S. Diploma or equivalent required; degree or college-level coursework in healthcare preferred.
Additional Requirements
The working hours are 8am-5pm, Monday through Friday.
